<p><span style="font-size:14.0pt">You are invited to help identify priorities for the next State Health Improvement Plan (SHIP). The SHIP identifies key health concerns for our state– along with strategies that will lead to improvements.</span>
<span style="font-size:14.0pt">A community based committee, called the PartnerSHIP</span><a href="healthoregon.org/2020ship"><span style="font-size:14.0pt">, has identified 14 strategic issues</span></a><span style="font-size:14.0pt"> that need to be addressed.
We need your help to understand which issues are most important to your community. There are a number of ways you can provide your feedback and help spread the word:

networks via email listservs, social media platforms and other communication channels. <b>We are especially interested in hearing from people and communities that experience health disparities. Please consider ways to share this survey with clients, consumers,
