Adam Leonard, MS, MPH, RN, CPNP, AAHIVS

Adam Leonard is a certified primary care pediatric nurse practitioner and certified HIV Specialist with over 10 years of experience working with adolescents and young adults in settings ranging from the San Francisco Department of Public Health safety net youth clinics to the Adolescent and Young Adult Medicine clinic at UCSF. He is active within the Society for Adolescent Health and Medicine, having previously served as a local chapter co-president. He currently serves as the co-chair of the organization’s Adolescents with HIV special interest group and is a member of the STI/HIV Health Services Committee. Adam is a PhD candidate at the Johns Hopkins School of Nursing, where he is researching the impact of youth-friendly health services on infectious disease outcomes.



 




CME Accreditation Statement



The AAFP has reviewed STI & PrEP Forum 2025-2026, and deemed it acceptable for AAFP credit. Term of approval is from 10/01/2025 to 09/30/2026. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

RE: Announcement: Gilead Sciences Awards NASTAD $5 Million Relink HIV Grant

NASTAD is pleased to announce that we have been selected by Gilead Sciences as the Lead Organization to implement our proposed Pathways to Care: A Relink HIV Initiative. This $5 million, two-year national initiative represents a major investment in strengthening re-engagement in HIV care for people with HIV (PWH) who have fallen out of care through equitable, data-driven, and health department–led approaches.

Through this award, NASTAD will support up to 15 organizations nationwide, providing subgrants ranging from $175,000 to $300,000, with the ability to request additional funding based on program scope, population size, and demonstrated need. The initiative is designed to strengthen state, local, and community capacity to identify, contact, re-engage, and retain people with HIV who are not currently in care by integrating surveillance-informed strategies, patient navigation, and cross-system collaboration.

The Pathways to Care initiative will:

  *   Support jurisdictions and their partners to implement scalable, community-centered re-engagement strategies aligned with Ryan White Part B, ADAP, and Ending the HIV Epidemic (EHE) efforts.
  *   Provide coordinated grantmaking, technical assistance, evaluation, and peer learning to strengthen data-to-care infrastructure and re-engagement workflows.
  *   Establish a national learning network to share best practices, advance equity-focused approaches, and promote long-term sustainability beyond the grant period.

NASTAD will leverage its decades of experience supporting HIV programs along with its robust grants management and compliance systems. This initiative builds on NASTAD’s broader commitment to strengthening health systems, advancing equity, and improving outcomes for communities disproportionately impacted by HIV.

In the coming weeks, NASTAD will release a Request for Proposals (RFP) to identify organizations interested in participating in the Relink HIV Initiative. Members are encouraged to review the forthcoming announcement to assess their interest in applying and share the announcement through their communication channels.
