2006-2007, dozens of partners committed to physical activity and nutrition came
together to develop a single comprehensive plan aimed at improving the health and
well-being of all Oregonians. The result is the "Healthy Active Oregon:
Statewide Physical Activity and Nutrition Plan 2007-2012" (the PAN Plan).
Many of you may have contributed to the creation of this Plan.<o:p></o:p></span></font></p>
<p><font size=3 face="Times New Roman"><span style='font-size:12.0pt'>Today,
the PAN Plan Steering Committee is sending you this survey to get your input
and feedback about prioritizing the objectives included in the PAN Plan. Please
complete this survey to share your priorities for physical activity and
nutrition in <st1:place w:st="on"><st1:State w:st="on">Oregon</st1:State></st1:place>.
The priorities identified through this process will provide us with a roadmap
for developing a one-year operations plan with targeted strategies to
