Dear Accreditation Work Group,

The agenda for our upcoming AWG meeting this *Monday, December 12th
1:30-3:30pm* is attached.

There are many useful presentations planned for this meeting, including a
discussion about the PHAB accreditation timeline, Community Health
Assessments, the MAPP Process and Hospital/ Health System Assessments.
