[LHDAccreditation] CLHO Accreditation Webinar Feedback-- Please Respond

Local Health Departments,

The next CLHO accreditation webinar is coming up on *Tuesday June 5th at
9am.* I will send out the registration information soon, but some feedback
as I am planning and preparing for these webinars would be very helpful.
Please look at the three following requests/ questions and respond as soon
as possible. If everyone provides feedback, I can make these webinars as
helpful as possible.

**1. **Do you have a piece (or more) of documentation that you would be
willing to share?*
If I get sample documentation, we will do some mock documentation scoring
during the webinar. This may help you find weak spots in your documentation
or you may find that the documentation lines up with all the requirements.
This will also help other accreditation coordinators get examples of what's
being used at other LHDs.

**2. **Are there specific areas within Domains 1 through 4 that you would
like me to focus on during this webinar?*
Do you have problematic measures or specific documentation questions, etc?
Please send these as soon as possible so I can prepare for them.

*3. Is there anything specifically that would be helpful for you in a
measure-by-measure documentation checklist?*
I am preparing documentation checklists, please send any specific needs you
have, for example: is it helpful for you to have a checklist as you gather
and review documentation, does it need to be something you can send out to
staff with documentation requests, is a checklist format helpful to you,
would another format be more helpful, etc?
