[LHDAccreditation] Updated Accreditation Readiness Timeline

Accreditation Work Group,

I am attaching an updated version of the accreditation readiness timeline
we created last year (thank you Matt for helping update this). We made some
changes based on new information from PHAB and tried to make it easier to
navigate.

Please feel free to adapt this and put in your own health department's
information. Let me know if you would like the publisher version so you can
make changes to the actual document.
