The proposed standards and measures for accredited health departments'
reaccreditation will be different than the requirements for initial
accreditation documentation. Rather than the identification and submission
of many documents of evidence, reaccreditation will require that health
departments provide narratives that describe their processes and
activities. Accredited health departments will be required to upload a
limited number of documents for reaccreditation.
The elements of the proposed standards and measures for reaccreditation are:
1. Continued conformity with the PHAB standards and measures under which
the health department was initially accredited.
2. Conformity with new requirements in standards and measures that PHAB
adopted after the health department received initial accreditation.
3. Updates and improvements on six documents: the community health
assessment, the community health improvement plan, the strategic plan, the
emergency operations plan, the workforce development plan, and the quality
improvement plan.
4. Continued improvement and advancement and quality improvement.
The reaccreditation Domains and Standards are the same as those for initial
accreditation. There is a link drawn between each of the reaccreditation
measures and the initial accreditation standards in the Standards and
Measures Version 1.5. This link keeps the standards the same for both
initial accreditation and reaccreditation and also demonstrates how
continued conformity with the Standards and Measures under which the health
department was accredited is assured.
The measures for initial accreditation have been reorganized and regrouped
for the reaccreditation requirements. Therefore a side-by-side comparison
of the proposed reaccreditation requirements and the initial accreditation
Standards and Measures will not be beneficial and will not provide clarity;
we do not recommend that you try to conduct such a comparison.
In addition, health departments seeking reaccreditation will be asked to
report on a selection of community health status outcome indicators. A set
of outcome indicators will be provided from which health departments will
select a subset for reporting. These are not provided here for your review
with this material because they are under development and will be used only
for reporting and not for formal assessment. PHAB is using such references
as the Institute of Medicine's *Vital Signs* Report; the National Quality
Forum's *Population Health Framework Action Guide*; the Robert Wood Johnson
Foundation's *Culture of Health Framework*; and other similar national
population health improvement outcomes measures. PHAB envisions that this
section will be a combination of a checklist menu (similar to the programs
and services menu that health departments completed in their initial
accreditation application), as well as some details on progress, similar to
what health departments have provided in their annual reports.

* Reaccreditation Requirements Development Work Plan *
Reaccreditation requirements will be finalized and adopted by PHAB's Board
of Directors in December 2016 for release in January 2017. The development
of the e-PHAB reaccreditation module will take place during the first half
of 2017. e-PHAB will be available for the first group of accredited health
departments up for reaccreditation to upload required documents beginning
the second half of 2017.
When the final requirements have been approved by the Board of Directors,
PHAB will publish them along with the process for reaccreditation and the
reaccreditation fees.
