wondering if anyone has been successful in incorporating the CHANGE assessments
(part of the CDC’s ACHIEVE grant) into their Community Health Assessment. I know
there are a few other Oregon counties who have been ACHIEVE communities and I’m
wondering if you’ve been successful in using that data as part of a broader
Community Health Assessment. I know that the CHANGE assessments do not
sufficiently meet PHAB’s definition of a CHA but it seems like a lot of the
information and partnerships could be coordinated into MAPP.
