Accreditation Work Group,<br><br>Attached are blank self-scoring templates for each domain. These are intended for when you are reviewing and self-scoring your own documentation, if that is part of your process (these are adapted from the Marni Mason example she reviewed during the QI and Performance Management Workshop). These may be helpful during your self-assessment process in order to identify if your documentation meets the PHAB requirement, gaps in your documentation and to prioritize improvement areas. <br>
<br>Each excel document has multiple sheets for each standard (see the tabs along the bottom of the documents). Each highlighted row is for a required accreditation document for Local Health Departments. There are extra rows if more than one example is required by PHAB.<br>
