<p class="MsoNormal">Attached in this e-mail is the process proposal document that I put together after our December 2018 partner listening meeting. This then got shared with our Steering Committee and Jefferson Regional Health Alliance (JRHA) board in January.
I based some of the specifics of our approach on Maricopa County’s approach in their first CHIP (I got connected with them through a MAPP training) as I felt that it fit with the varying levels of commitment and capacity around our partner table for engaging
