[lstac] Site Visit Form that is fillable?

Hi All--

Maryland has an IMLS site visit coming up and I can't help but notice that
the form that IMLS provides is wonky. It doesn't quite convert correctly to
a word doc and even when I open it in Adobe Acrobat it is still a little
weird and unaligned.

Is there anyone who has re-created this form to be more easily completed?
