[A person wearing glasses Description automatically generated with low confidence]As children across Oregon begin the new school year, it is a reminder that we all have a responsibility to support the health and well-being of the children and families in our communities. When we come together to support children and families and make sure that they are safe and that their basic needs are met, we help them grow and reach their human potential.
Educators, who are mandatory reporters of child abuse and neglect, have a unique opportunity to get to know the children and families they see daily. They are there to share and experience a child’s victories, strengths, and challenges as they learn, grow and interact with friends at school.
As mandatory reporters, when educators have a reason to believe a child is experiencing abuse or neglect, they are required by law to report that concern to the Oregon Child Abuse Hotline by calling 1-855-503-SAFE (7233).
But what should an educator, or other caring adult, do when they see a child and family who is in need or crisis, but do not believe that a child is being abused or neglected? Many families in Oregon are still struggling to meet their basic needs, because of the impacts of COVID-19 and the rising costs of food and housing. These are families that need help getting connected to the resources available in the community, instead of being reported to the Oregon Child Abuse Hotline.
Our data tells us that many people turn to the Oregon Child Abuse Hotline when they want to help and support a child and their family but are not sure how.
This year approximately 42,000, about 45%, of all calls to the Oregon Child Abuse Hotline were not reports of suspected abuse or neglect.
These are calls from educators and other members of our community who recognize that a child and their family might need assistance because they are experiencing poverty, hunger, housing instability, job loss, mental or physical health issues, or other stressful situations.
Often these calls come to us because the callers are not sure where to turn for support.
There are many different resources and supports available in our communities to help children and families meet their needs. Sometimes, the best way to support child and family wellbeing and safety is to help them get connected to the resources they need:

We all want children and families in Oregon to be healthy, safe and happy. Child abuse and neglect is preventable when communities come together to support children and families, and there are many organizations, nonprofits, and people who can help support families and prevent a family’s crisis from becoming a child’s safety issue.

Children’s advocacy centers (CACs) are uniquely positioned to support a child’s recovery from the psychological and emotional effects associated with trauma. However, not all mental health providers or mental health treatments are equal or equally accessible. Children and youth served by CACs need and deserve access to providers skilled in trauma treatment. The National Children’s Alliance’s National Standards of Accreditation have moved assertively in recent years to require CACs to employ or partner with providers trained in select evidence-based trauma treatment models. Meeting this expectation is challenging even in the most resource-rich communities, but for CACs serving largely rural and even frontier regions (where less than 6 people live per square mile), the challenge can be more daunting. Qualified providers may not be within reasonable driving distance for children and families living far from population centers, or providers may be overwhelmed by referrals and children must wait months to initiate therapy.

Given this context, the Western Regional Children’s Advocacy Center (WRCAC) partnered with the Children’s Alliance of Montana (CAM) and Children’s Advocacy Centers of Washington (CACWA) to pilot efforts to build telemental health networks across their respective states. This issue features important lessons learned along the way in Montana. The vision was to implement a “hub and spoke” model, based on an idea pioneered by the Veterans Administration over 20 years ago to serve Native American veterans with PTSD living in remote regions of the northern plains1. Our adaptation of the model envisioned linking a network of trained trauma therapists in urban areas (the “hubs”) to children and CACs spread around the state in rural and frontier areas (the “spokes”) through telemental health service delivery. The National Child Traumatic Stress Network’s online Trauma-informed Organizational Assessment Course<https://www.nctsn.org/resources/nctsn-trauma-informed-organizational-assessment-course?utm_source=ebulletin&utm_medium=email&utm_campaign=nctsn-ebulletin> helps organizations assess their current practices in the context of serving children and families who have experienced trauma. It is an important part of an organizational transformation process to create trauma-informed organizations. The NCTSN TIOA is a process created by the UCLA-Duke University National Center for Child Traumatic Stress (NCCTS). This course includes the assessment and accompanying resources for professionals interested in using the TIOA and provides information to get started, access to the assessment, guidance on using the assessment, and resources for implementation and organization change.

OVERVIEW
CCAR focuses on the field response and covers a range of topics that are essential knowledge for anyone engaging with families where child abuse is suspected. This training is recommended for all members of the multidisciplinary team and fulfills statutorily mandated training requirements. CCAR focuses on field response and is appropriate for all members of the MDT. Participants may include law enforcement, child protective service workers, school officials, medical professionals, mental health treatment providers, Children's Advocacy Center employees, juvenile department representatives and others (ORS 418.747).
PILOT TRAINING EXPECTATIONS
As part of the pilot process, participants must commit to attending both days of training, complete pre-and-post knowledge tests, submit evaluations after each session and submit one follow-up evaluation on the effectiveness of the training in improving practice. Cancellations must be made 10 or more days prior to the start date of the training.

Both these resources shed light on the health, economic, education, community, and environmental challenges impacting children in Oregon.
Oregon findings show that many indicators of child well-being saw some improvement: the rate of children in poverty fell from 21% to 15%, children living in families where caregivers were struggling to maintain steady employment dropped from 34% to 27%, and families burdened with spending 30% or more of their income on housing went down from 43% to 31%.
Amidst these improvements, however, the mental health challenges among Oregon youth have soared. Data show over 117,000 Oregon children struggling to make it through the day as effects of the coronavirus crisis linger. Between 2016 and 2020, the rate of Oregon children ages 3-17 who had experienced anxiety and depression during the first year of the COVID-19 crisis rose from 11% to 16.1%, exceeding the national average of 11.8%. Further, nearly one-in-six Oregon youth saw their needs for emotional or mental healthcare go unmet during the pandemic as they couldn’t access a healthcare provider.

“These positive shifts, however small, show that what we do as advocates, state leaders, and community members matters. We know what it takes to have healthy, thriving children and youth,” said Jenifer Wagley, executive director of Our Children Oregon.

The pandemic years have taken a toll on everyone. Economic uncertainty, including housing stability, has had an outsized effect on many children and families experiencing financial challenges, leading to increased anxiety and stress. The mental health challenges brought by the pandemic have compounded conditions that made life harder before the coronavirus, with disparate challenges affecting children of color and of other marginalized identities, abilities, and backgrounds.
“There are many nuances regarding youth mental health in Oregon and the United States, and each data point only tells part of the story. Gender identity and race and ethnicity play major roles in facing systemic and interpersonal marginalization, accessing quality mental health services, and navigating stigmas around accessing this care,” shared Jyoni Tetsurō Shuler, research manager at Our Children Oregon.

Mental health is a critical part of the overall health of children and youth, impacting all facets of their lives in and out of school. Children have their best chance of thriving when they can learn skills to manage anxiety and benefit from quality mental health care services that they can access easily and without stigma, and that meet their unique experiences and identities.
Our Children Oregon joins the Casey Foundation in calling for lawmakers to heed the U.S. surgeon general’s warnings and respond to our youth mental health crisis by developing programs and policies to ease mental health burdens on children and their families.
