fill out and submit electronically. This takes the place of the link that was previously sent. You can submit using any electronic device (including IPads). You will submit through our CVSD website<span style="color:#1F497D"> at

</span>. <b>Please do not use the old link previously sent.</b> There will still not be the capability to upload reports at this time and you will still need to email those reports to the CVSD email like you all have been doing.

date of birth. The other new feature is that there will be a checkbox for the victim/applicant to mark along with printing their name which will become their electronic signature. The signature page will no longer need to be submitted in paper form. If you
