Dear Business Managers and Benefits Administrators –

We have added some new forms to the website for the 2014-15 plan year!


On the Member Forms<http://www.oregon.gov/oha/OEBB/Pages/forms.aspx> page, you can find:

​ ● 2014-15 New Hire Enrollment Form<http://www.oregon.gov/oha/OEBB/docs/Form/2014/Enrollment2014-15.pdf>
Use this form to initially enroll in benefits effective October 1, 2014 or later.


On the Entity Forms & Reports<http://www.oregon.gov/oha/OEBB/entity/Pages/eeforms.aspx> page, you can find:

● 2014 Open Enrollment Change Form<http://www.oregon.gov/oha/OEBB/docs/Form/openenrollment.pdf> (available here for entity planning purposes - will be on "Member Forms" page August - September)

● 2014-15 New Hire Enrollment Form<http://www.oregon.gov/oha/OEBB/docs/Form/2014/Enrollment_rev061614_customizable.doc> (Customizable for your entity - Microsoft Word format)
