To our Valued Benefits Partners:

Similar to last year, OEBB will be distributing the 2015-16 Open Enrollment Guide directly to members along with a cover letter listing the specific plans available to them. This year we have also created customizable PDFs.

You can use the links below to download customizable PDF documents should you choose to customize and print your own booklets for the upcoming 2015-16 Open Enrollment period. The customization options will allow you to exclude some information about plans that may not be available to your employees.
Please Note:
1)      Only select sections are available for customization -- see the "Instructions for Customizing Booklets" document below for more details.
2)      OEBB intends to send a full booklet to each benefits-eligible employee or early retiree, based on the plans available to their employment group -- see the links and descriptions below for more detail. If you would like OEBB to exclude your employees from this mass mailing, you must send an email to jackie.cowsill at oregon.gov<mailto:jackie.cowsill at oregon.gov> by close-of-business Thursday, July 9, 2015. If you would like OEBB to bulk-ship your members' cover letters and envelopes to your entity so you can assemble them into packets with your customized booklets, please indicate this in your email request.
3)      Entities choosing to customize their booklets will be responsible for their own printing and any postage beyond the bulk-shipment of materials to the entity office. OEBB cannot print or mail booklets that are more customized than the three full versions identified and described below.

Customizable Booklet for Kaiser Area<https://www.oregon.gov/oha/OEBB/Communications/KMHSS-2015-Custom.pdf> (all plans included) - OEBB will send a complete version of this booklet to all members with at least one Moda Statewide, Moda Synergy/Summit and Kaiser medical plan available.
Customizable Booklet for Non-Kaiser Areas <https://www.oregon.gov/oha/OEBB/Communications/MHSS-2015-Custom.pdf> (includes Moda Statewide and Moda Synergy/Summit plans, but no Kaiser plans) - OEBB will send a complete version of this booklet to all members who have no Kaiser plans available, but do have at least one Synergy/Summit plan available.
Customizable Booklet for Moda Statewide Plans only <https://www.oregon.gov/oha/OEBB/Communications/MH-2015-Custom.pdf> (no Kaiser or Synergy/Summit plans) - OEBB will send a complete version of this booklet to all members who have no Kaiser or Synergy/Summit plans available.
