<p class="MsoNormal">Just a quick reminder that Evidence of Insurability (medical history/underwriting) Forms must be completed personally by the employee applying for coverage. In our sometimes complex world of benefits, it may be tempting to “assist” an employee
by completing the form for them. Keep in mind, however, this form asks for HIPAA-protected personal health information (PHI) and no one other than the employee and the affected insurance carrier (The Standard or Unum) should be involved in this part of the
process. Also, you could actually be held liable for fraud if the information provided on the form is incorrect or incomplete.<o:p></o:p></p>
