<p class="MsoNormal">During the 2015 Open Enrollment period, members were able to enroll in a Healthy Futures medical plan and begin receiving the Healthy Futures incentive (reduced deductible or copays) effective October 1, 2015, if they and their enrolled

 spouse/partner (if applicable) committed to complete their medical carrier’s online health assessment by October 15, 2015. When members enrolled in this option, they acknowledged that if they did not complete the health assessment requirement by October 15,

 2015, their plan would change back to the regular non-incentivized plan retroactively effective October 1, 2015.

<p class="MsoNormal">Due to technical difficulties experienced by some Moda members attempting to complete their health assessment, OEBB is allowing members who established a myModa account by the October 15<sup>th</sup> deadline (the first step required to

 access the health assessment) to be credited as completing the health assessment and will retain their Healthy Futures incentive for the remainder of the 2015-16 plan year.

Note: These members will still need to log into the MyOEBB system during the 2016 Open Enrollment period and report their healthy actions in order to remain eligible for the Healthy Futures incentive in 2016-17.

<p class="MsoNormal"><b>This past weekend, members enrolled in a Moda Healthy Futures plan for 2015-16 who did

not take this first step to create a myModa account had their plans changed back to a regular non-incentivized plan retroactively effective October 1, 2015.

</b>OEBB will mail the attached letter to the affected Moda members this week (target mail date Wednesday, December 16<sup>th</sup>).<o:p></o:p></p>

<p class="MsoNormal">If your entity has any affected members in Moda plans, you currently have a list of these members in your document management area of MyOEBB. The file is named “Missing_HA_Report_Entity_Name_121215”.
