<P><FONT FACE="Times New Roman">This message is intended only for individuals who deal with the ODOT Material Laboratory Reports and Sample Data Sheets (form 4000).</FONT></P>


<P><FONT FACE="Times New Roman">Effective Tuesday July 1, 2008 the final laboratory report will no longer have a signature stamp.  Instead the final laboratory report will have "Kevin Brophy - Laboratory Services Manager" or "Keith Johnston - Structural Services Engineer" appearing on the lower section of the report.</FONT></P>


<P><FONT FACE="Times New Roman">Effective immediately I request that all Sample Data Sheets (form 4000) include the Quality Control Compliance Specialist's name, contact number, and certification number in the "REMARKS/SPECIAL REQUIREMENTS" section of the form.  This will allow us to more effectively address our customer's needs. </FONT> </P>
