[OEBBpremiumcontact] 4th Quarter EAP Counts

[For educational entity personnel responsible for making premium payments to OEBB.]

Good morning,

It is time once again to submit your EAP counts for the 4th quarter (July-September) of the plan year 2013-14.

Good news, you no longer have to submit to us a list of your Non-Oebb member's names.  Just list the count of your Non-OEBB members on the attached word document and return to me no later than June 20th.  If you have any questions, please feel free to contact us.  Thank you.
