October 2025 Monthly Billing Statement Issue

It was brought to our attention that some may have received Monthly Billing Statements that have an incorrect EAP premium.



This issue has been resolved and New Statements were emailed tonight around 11:00pm on October 2, 2025. Please do not use the emailed statements that went out this morning.
