<p class="MsoNormal"><b>*Monthly Billing Statement – </b>Will be sent Thursday, <b>
May 1<sup>st</sup></b>, this includes current billing and any changes made in April.<o:p></o:p></p>
<p class="MsoNormal"><b>*Retroactive Report –</b> Will be uploaded to document management Thursday,
<b>May 1<sup>st</sup></b>. This is a per member detailed report of retro changes.<o:p></o:p></p>
