<p class="MsoNormal">I have had a couple questions asking if there was a list of questions for each nomination category that they could work off of before copying and pasting into the survey monkey nomination form. Attached is the list of the nomination questions
for each category. You still need to submit your nominations through survey monkey but hopefully this helps those that like to work offline before inputting into online forms.
