[OPEUreppms] Information Request (801 Forms)

In accordance with the Public Employee Collective Bargaining Act
(PECBA), SEIU Local 503, OPEU is requesting the following information,
specific to the SEIU bargaining members, to prepare for negotiations.  

 

1.        The number of on-the-job injuries as measured by the
completion of 801 forms between July 1, 2009 and January 30, 2011, by
agency. 

2.       The total number of compensatory SAIF claims lasting longer
than fourteen (14) days between July 1, 2009 and January 30, 2011.

3.       The total number of compensatory SAIF claims lasting fewer than
fourteen 914) days between July 1, 2009 and January 30, 2011.

 

If you track the above information electronically please forward to me
by March 11, 2011.   If it isn't electronically and will take time to
get, please provide what the estimated cost, including staff time and
production.  Preferably before acting so we can tell SEIU what it's
expected to cost to produce the data .
