Attached please find the official SEIU/OPEU 2011-2013 Contract Order
Form.  The previous email inquiry was a request for an estimated number
of contracts your agency would need, and will not be considered the
official order request.  

 

Please fill out the attached form and return it to me by email, hard
copy or fax no later than September 23, 2011.

 

Again, we would like to encourage use of the electronic version of the
CBA whenever possible.
