I have a friend who works for one of the counties in Oregon as a GIS staff person.  He was involved in converting addresses to the new 5 digit addresses for emergency services.  You might try calling the county and inquire if they have a database of old addresses in order to get the new one. There are web sites (I found a few just based on your statement) but they charge for their services. If you only have the one address I would call the county and see if they can help you. I would think that if they can't they can point you in the right direction.  You also might check with the U.S. Post Office to see if they may know.  I bet the mail carrier who has this route can tell you if he/she had this route before the addresses were converted.
