<div>Are you talking about the persons original Social Security application? If so, when I got my grandmother's it was wonderful. The best document I ever got. It had her real name, she always went by an alias and I didn't know what her real name was and it also gave her place of birth, which was rural Grant County, Oregon. It had a picture of her and what work she was doing at the time.</div>
