[ORDiabCoalition] Update - Oregon Health Improvement Plan Committee

Dear Partners,
Last December you received an email requesting nominations for the Oregon Health Improvement Plan Committee. The committee has been charged with creating an action plan focusing on best and promising practices known to prevent and manage chronic disease across the lifespan, from infancy through adulthood. The plan will be an action strategy that expands beyond medical care to improve the health of Oregonians at the statewide and community levels. The plan seeks to acknowledge and build on the experience, contributions and successes of organizations and communities across the state.
The Oregon Health Improvement Plan Committee has begun its work and is in the information gatherings stage. To find out more about the plan, view meeting materials, and obtain meeting dates and locations, please visit the Oregon Health Improvement Plan website at:
