<div><font color="#1F497D">PEBB has implemented a new surcharge for 2021 plan year. If a spouse, domestic partner or a dependent child is enrolled in a medical plan (except opt out) for 2021 and is also enrolled in a PEBB or OEBB medical plan as a subscriber,
this new $5 surcharge will apply. The OSPA deduction code for the double coverage surcharge is DBSC. The plan codes are AAA for pre-tax and 111 for post-tax. This has fed over during the open enrollment process<font color="black"> </font>for the employees
