Attached please find a form requesting information on the total number of W-2s  issued by your entity for employees for the year ending December 31, 2011.  

Annually, this information is gathered for the computation of the yearly Oregon State Social Security administration fee. The statistics supplied by you  for the year ending 2011 will become the basis for the fee paid in 2013-2014 by your political subdivision.  Please return this form at your earliest convenience via email, fax, or by mail.
