<p class="MsoNormal">The Oregon Department of Education (ODE) has recently revised the Type 20 Activity Driver Application Form 581-1276-M to comply with OAR 581-053-0420 (5) (b), which states:<o:p></o:p></p>

<p class="MsoNormal"><i>Type 20 Certificates expire: (a) two years after the date issued by ODE for 11-14 passenger certificates; (b) on the date of medical certificate expiration for 15-20 passenger certificates.<o:p></o:p></i></p>

<p class="MsoNormal">The newly revised form can be found online at <span style="color:#1F497D">

<p class="MsoNormal">We will continue to accept the old version of this form for 11-14 passenger certificates until November 1, 2014. 15-20 passenger certificates will need to be submitted on the new version starting immediately.

<p class="MsoNormal">Please be aware of the type of certificate your driver may need. Many vehicles are advertised as 15 passenger vehicles, but this number includes the driver. ODE considers these vehicles to be 11-14
<u>passenger</u> vehicles, and the Type 20 Form should be marked accordingly. <o:p>
