You should note that the form has changed this year. The form collects information for the 2009-10 and 2010-11 school years, and is due no later than October 1, 2010. These documents can also be found on the ODE web site at http://www.ode.state.or.us/search/page/?id=1743. This is a requirement of IDEA 2004. All districts and agencies using IDEA funds must report annually to the state (a) the number of students who received Coordinated Early Intervening Services, and (b) the number who received Coordinated Early Intervening Services and subsequently received special education and related services during the two year period after receiving Coordinated Early Intervening Services.
