Piloted county -wide, this new SSD Online system efficiently streamlines requests, provides management tools, and eliminates the need for paper forms-- understandably, very popular. SSD Online provides the following key capabilities: a. See all requests in one place b. Simply submit requests online c. Check on the status of past and new requests d. Detect and correct errors to dramatically reduce the likelihood of submission errors e. Notify about needed documentation f. Print out rosters at any time. The College Board is prepared to offer a high level of support, including webinars for school-site staff who will be responsible for requesting accommodations. SSD coordinators at all schools were sent access codes to enable them to log into the system. They must first have a College Board account. Access codes can be obtained by calling the SSD Program at 800-257-5123. The process follows this course: 1. SSD coordinator obtains collegeboard.com account and online access to the system. 2. Log onto SSD Online to begin the process. 3. Print out a parental consent form, have the parent sign it, file it at school. 4. Complete questions online regarding the student's disability, requested accommodations, and available documentation. 5. Submit request electronically. 6. The system will inform if documentation is required. 7. When required additional documentation can be mailed or faxed in. 8. Receive e-mail notification that decision has been made; log in to view decision letter. 9. Log in at any time to see status of students' request or to print a roster. I wanted you to know that the request had been made: To: All Vendors of Electronic IEP Applications: This is a request to change an item on the Oregon IEP form that you maintain for some districts. I spoke to the Special Education Director Advisory last week and they agreed that we needed to get this request in early and hopefully you will make the change to the IEP form in a timely manner. I know that you have up to a year from this request to complete the task. The change on the IEP form has to do with a decision made relative to assessment and accountability. Starting in the 2010-11 school year, federal high school accountability will be based on the number of students who pass state assessments by the end of their 11th grade year. High School students may still take state tests in 9th and 10th grade, but all 11th grade students who have not previously passed a state test must take that test at least once during their 11th grade year. As a result of this change, ODE requests an update to the existing IEP form. The request is to change the assessment page of the IEP and any related references to the statewide assessment in High School. References to statewide assessment at 10/CIM should be changed to reflect statewide assessment at HS (11th) as appropriate. Attached to this message for your convenience is a copy of the current IEP form and guidance - see page 6 (guidance) and page 13 (actual IEP form). I will also send this request to the ESD and district directors of special education and to IT managers and appropriate Data Collectors so they are aware.
