Attached is a grant application for The Transition from School to Life Grant.  The Transition from School to Life Grant was established by the passage of the 2013 Legislative's HB3264. The purpose of this application is to provide districts the opportunity to access grant funds made available through the Transition from School to Life Grant and to solicit proposals for the grant that will advance transition support for students with disabilities.
