As a leader of a local education agency special services program, you are being asked by the Department of Human Services Employment First Initiative to complete a short survey that will help to determine how to best support local leadership in the implementation of the Employment First Initiative and policies.
Time Commitment - It is estimated this survey will take approximately 10 minutes
Purpose - The purpose of this survey is to gather data from INDIVIDUAL LEADERS of local developmental disability program/agencies, local education agencies, and local vocational rehabilitation offices regarding their perspectives and participation in local Employment First Teams or other Cross-Agency forums in which the Employment First Initiatives and policies may be a primary or regular agenda item of discussion. The data will be used to better understand the role and participation of local leadership in local Employment First Teams and Cross-Agency forums, and how the Department of Human Services (DHS) and Oregon Department of Education (ODE) can best support local leadership in local collaboration efforts to further implement the Employment First Initiative and policies.
Deadline - please complete this survey within two weeks, by Thursday August 27th, 2015.
