The Statement of Eligibility form for the category of Specific Learning Disability has been revised, along with instructions for completion of the form. Form revisions assure alignment with both best practices and federal guidance for the identification of a Specific Learning Disability.

The documents were revised to align Oregon forms with Title 34, Code of Federal Regulations, section 300.307, which provides that states must not require the use of a severe discrepancy between intellectual ability and achievement for determining whether a child has a specific learning disability; must permit the use of a process based on the child's response to scientific, research-based intervention; and may permit the use of other alternatives.
