I am reaching out to request your support in sharing an important opportunity for Diabetes Self-Management Training (DSMT) providers in your state and networks. The National Association of Chronic Disease Directors (NACDD), in collaboration with the Centers for Disease Control and Prevention (CDC), is launching Year 2 of the DSMT Enrollment Pilot Project. This initiative aims to better understand barriers to DSMT enrollment, retention, and reimbursement, and to develop practical tools and technical assistance resources to support providers nationwide. We are seeking to engage a diverse group of DSMT providers, and your assistance is essential to ensure we reach organizations across different settings and communities.
