Here is the third item to share. It is an opportunity for your DSMT sites.  Please see below:


Hello DSMES State Specialists,



I hope you are doing well. I am reaching out to request your support in sharing an important opportunity for Diabetes Self-Management Training (DSMT) providers in your state and networks.



The National Association of Chronic Disease Directors (NACDD), in collaboration with the Centers for Disease Control and Prevention (CDC), is launching Year 2 of the DSMT Enrollment Pilot Project. This initiative aims to better understand barriers to DSMT enrollment, retention, and reimbursement, and to develop practical tools and technical assistance resources to support providers nationwide.



We are seeking to engage a diverse group of DSMT providers, and your assistance is essential to ensure we reach organizations across different settings and communities.



Project Overview:

  *   Timeframe: May - July 31, 2026
  *   Funding: $3,000 per selected organization
  *   Participation Includes:

     *   2-3 virtual interviews or small group discussions (1-1.5 hours each)
     *   Sharing insights on enrollment, retention, billing, and reimbursement
     *   Providing feedback on tools, templates, and technical assistance resources
     *   Contributing promising practices and real-world experiences



How You Can Help:
We would greatly appreciate your help in:

  *   Sharing this opportunity with DSMT providers in your state or network
  *   Encouraging organizations with diverse experiences (e.g., health systems, FQHCs, pharmacies, community-based organizations) to apply
  *   Helping elevate providers who may benefit from technical assistance and are willing to share their experiences
