The State of Oregon, acting by and through the Oregon Health Authority, (“Agency”), has issued a Request for Proposals to establish and operate the Oregon Self-Management Hub. The Hub is envisioned as an efficient, effective delivery and financing system for

 evidence-based chronic disease self-management programs (such as Living Well with Chronic Conditions). The Hub will coordinate statewide supply and demand for the programs by developing a delivery network, marketing the programs to payers and Participants,

 and administering centralized systems for payment processing, data collection scheduling, workshop leader training, and quality assurance.<br>
