<p class="MsoNormal">The National Association of Chronic Disease Directors (NACDD) is working with KDH Research and Communications (KDHRC) on a project funded by the CDC Arthritis Program to repackage the Exercise is Medicine (EIM) guidance as a way to promote
health care provider (HCP) counseling and referral of patients with arthritis to physical activity AAEBIs—arthritis appropriate evidence-based interventions. The activities associated with this project are intended to complement Strategy 2 in the 1803 NOFO.

<p class="MsoNormal">As part of this project, KDHRC is conducting <span lang="PT-BR">
market research via an <u>online anonymous survey</u> with healthcare providers (HCPs) to identify appropriate incentives to increase counseling and referral rates of AAEBIs. For the purpose of the project, HCPs are defined as doctors, including specialists
and rheumatologists, nurse practioners, physical therapists, and pharmacists. <b>We invite you to share this
<u>online survey link</u> with HCPS in your state who might be interested in providing input to help shape the launch of the revised programming.</b> The survey is short and should take less than 15 minutes to complete. The survey will be open for a brief
period of time to collect information needed for this phase of the project.<o:p></o:p></span></p>
