the new Oregon Health Plan (OHP) benefit coverage for these classes. Oregon Health Plan includes Medicaid Fee For Service and Coordinated Care Organizations. This survey is designed to learn more about how the fall prevention programs are offered and what
successes and barriers have been experienced. </span><span style="font-size:12.0pt;color:black;background:white"><o:p></o:p></span></p>

<span style="background:white">Results from this survey will be used to inform a landscape analysis Comagine Health is completing in partnership with the Oregon Health Authority to better understand current community capacity for these programs and infrastructure
needs that needs to be addressed to meet the demand of this new benefit in Oregon. The data will be used to inform a final report for the Oregon Health Authority, and it will be aggregated and deidentified before it is shared in that report. <o:p></o:p></span></span></p>
