The Health Evidence Review Commission (HERC) is looking at potentially expand coverage for CGM (continuous glucose monitoring) for people with diabetes on OHP. They have a survey they want to share with people on OHP who might want to use CGM to understand
better how using CGM might affect their quality of life.

The audience of this survey is: Oregon Health Plan (OHP)/Medicaid members or a caregiver/parent of an OHP member who uses insulin to manage their Type 2 diabetes. The survey is in Spanish and English,
below.

The timeline is tight, especially for reaching patients and caregivers. We believe this is because of a HERC process deadline and that it cannot be pushed out. If it is possible for you to ask any patients or caregivers to complete this, it could provide support
for expanded CGM coverage.

Thanks so much,
