[Libs-Or] Mobile Phone stipend

City of Sherwood has three tiers of phone stipends, depending on the situation. As Library Manager and because of the amount of work I do via my phone, I qualify for the 3rd tier. I have one supervisor who often arranges on-call shifts on the fly from home and qualifies for the 2nd tier. The form is pasted below.




REQUEST FOR CELLULAR PHONE STIPEND

Please accept this signed form as my request for a cellular phone stipend. Authorization for stipend is at the discretion of the Department Head.

CELLULAR PHONE STIPEND POLICY

Employees of the City of Sherwood may be eligible for a monthly stipend for use of their personal cell phone for City business. This is a taxable benefit and will be paid through payroll. In order to receive this benefit one of the following options must apply:

1.     $15 monthly stipend
*        Employee shall provide and use a cell phone as needed to conduct City business and
*        Employee's personal cell phone shall include voicemail features.
*        SPOA employees only

2.     $40 monthly stipend
*        Employee shall provide and use a cell phone as needed to conduct City business and
*        Employee's personal cell phone shall include voicemail features and
*        Employee agrees that the cell phone shall be used for City business, including being available for contacts by the City outside of regular business/shift hours

3.     $80 monthly stipend
*        Employee shall provide and use a cell phone as needed to conduct City business and
*        Employee's personal cell phone shall include voicemail features and
*        Employee agrees that the cell phone shall be used for City business, including being available for contacts by the City outside of regular business/shift hours and
*        Employee agrees to have access to City email on their personal cell phone

The City shall pay the employee ______ dollars per month for a cellular phone stipend to cover a portion of the employee's personal cellular phone bill to account for the portion, regardless of the actual cost, attributable to City business. Stipends will be paid on a monthly basis through payroll and shall be effective the first of the month following authorization and submittal to payroll.

I have read the Cellular Phone Stipend Policy and agree to the terms as stated.

Our District gives a $15/month stipend to staff for whom mobile phone use is considered "essential": Director, Assistant Director, and Outreach Specialist.
