[Libs-Or] Patron issues - documentation

Hi, all,

Does anyone have a fillable form you use when documenting patron issues? Something that you give a copy to the patron in question and keep a copy for your records. Something that lists the nature of the infraction and the length of suspension from using the library. Something like a rubric.

If so, I would appreciate you sending it my way.
