Living Well with Chronic Conditions Leader Training
Our community needs you! Become a Living Well with Chronic Conditions facilitator via this free leader training, developed by Stanford University, to help people manage chronic pain. This training is provided free for participants through support from Multnomah County, Aging & Disability Services and Friendly House. Once you are trained you will be able to provide this great service in local community-based organizations throughout our region. Each workshop is facilitated by two leaders. Know someone you work really well with? Recruit them to get trained with you! Only 20 spots available so submit your application ASAP!

As a Reminder:

  *   Program that is sponsoring participant must have a current CDSMP License with Stanford University.
  *   The application must be signed by the program coordinator that is sponsoring you to attend.
  *   Participants must attend all four days and complete two practice teaching segments to complete the training process.
  *   Ability to facilitate a full six-week session is required within 6 months of completing the leader training. A list of upcoming LWCC workshops will be provided on day four for you to select from.

What is the Living Well with Chronic Conditions workshop?



Living Well with Chronic Conditions is the Chronic Disease Self-Management Program, developed by Stanford University, It is an evidence-based six-week workshop that meets for 2.5 hours each week and provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Workshops are facilitated by two trained leaders, one or both of whom are non-health professionals with a chronic disease themselves. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about. Subjects covered include: 1) techniques to deal with problems such as frustration, fatigue, pain and isolation, 2) appropriate exercise for maintaining and improving strength, flexibility, and endurance, 3) appropriate use of medications, 4) communicating effectively with family, friends, and health professionals, 5) nutrition, and, 6) how to evaluate new treatments. Each participant in the workshop receives a copy of the companion book, Living a Healthy Life With Chronic Conditions.  It is the process in which the program is taught that makes it effective. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. Chronic Disease Self-Management programs are supported by the Oregon Health Authority and the Oregon Department of Human.
