[Self-Management_OR] Upcoming CDSMP and CTS Trainings

Hello everyone, 

We will be offering a Chronic Disease Self-Management training and a
Cancer: Thriving and Surviving cross training in the near future! Note
that in order to take the Cancer: Thriving and Surviving training,
participants must have completed the 4 day Chronic Disease Self-Management
training. Please see the information below and the attached flyers and
applications.
