[ASIST_Network] Please read: Correct Forms for ASIST NASW CEUs

Dear Oregon ASIST Trainers,
Thank you for returning the rosters and evaluation forms for Oregon NASW CEUs to me following your ASIST trainings - it's great to see so many trainings taking place and participants able to get CEUs for them!

One problem - are you using last year's forms?  The "Conference Number" on the form is assigned by Oregon NASW and it changes each year (mid-July for us) and I've been getting last year's forms from some of you. Please discard/delete all your old forms and use the attached ones. The correct "conference number" is  19-14-145  - please use that number. I'll change the number for you on the evaluations and rosters you've sent me, but your participants will need to have the correct number on their certificates to align with whatever reporting they do to Oregon NASW to obtain the CEUS. You'll need to notify them and get that change made.

One more thing, would you please fill in the "Conference Date" on the forms? That is really helpful too.
