[ASIST_Network] Templates for MoU with schools?

Hello,
I am the Youth Suicide Prevention Coordinator in Jackson County. We recently composed a letter of agreement with an entire school district and their school based health centers. that also has a re-entry protocol for when a suicidal student returns to school. See attached for more details. The EPSD flow chart shows the process for which JCMH and the school district communicate and respond, and the LOA is the written portion of exactly how we work together and what agreements we maintain between JCMH, Rogue Community Health, and Eagle Point School District. I try to meet with each district at the beginning of the school year and modify each flow chart and letter to both reflect the schools need, while keeping best practices in mind. On of the NW Tribal clinics currently implementing ZS is hoping that someone has a template or an example of a MoU between a local school and your clinic describing the referral process the school will begin if and when a student presents with suicidality.
