[Cnaprogram] Obtaining copies of lost or misplaced nursing assistant or medication aide training program certificates of completion

Hello:

The primary source for replacing lost or misplaced nursing assistant or medication aide training program certificates is through the training program. Training programs in Oregon are required to keep copies of certificates of completion for seven years.

If it has been over seven years, the individual may submit a public records request to the Board via the process found at http://www.oregon.gov/OSBN/Pages/online_records.aspx .  There is no guarantee that the Board has a copy of the certificate of completion. However, if the Board kept a copy when the original application was submitted for certification, this may be an option for obtaining a copy when the original certificate has been lost or misplaced.
