[Cnaprogram] Applying for a nursing assistant or medication aide examination retake

Hello:

Please use the Re-examination Application form found at http://www.oregon.gov/OSBN/pdfs/form/LIC-703B.pdf when applying to retake the CNA or CMA exam.  Do not fill out another initial exam application (form LIC-703 or LIC-704).  Submitting another initial application slows the processing of the retake application.

You may also just call the Board's main phone number, 971-673-0685, and pay the retake fee to expedite getting released for a new exam date. Calling a licensing technician directly takes that individual away from processing exam applications and releasing applicants for testing.
