Tuesday Tidbits for D5 Child Welfare staff, Oregon Eligibility Partnership staff, Self-Sufficiency staff, and Community Partners!

Tuesday Tidbits are short, specific info bites you can use and share to increase collective knowledge of Lane County's wide-ranging community resource landscape.

WHEN: September 30th, 9:30am - 12pm
WHERE: PacificSource, 555 International Way, Springfield

This training is offered by 15th Night.

Capacity is limited! 2 Reps per agency/organization.

Direction Service Wraparound<https://www.directionservice.org/> is a process of individualized care planning and coordination for youth with complex needs and their families that proceeds through four phases and is guided by 10 principles.

Eligibility requirements:

  *   Aged 0-20
  *   Multi-system involvement of the youth (at least 2)
  *   Complex needs that can beenfit from a team approach
  *   Consent from the parents and youth (if 14 or over) to participate. It is a voluntary process
  *   Enrolled in OHP
